Patient Rules for the Alborzi Exchange Reward Program!

You can earn TOKENS at your regularly scheduled visit (except, of course, emergency appointments)!
 
It all starts with GOOD Oral Hygiene!
You must have good oral hygiene at your visit to participate.
Here is How it Works!

If you are IN Braces:
  Earn up to 4 Alborzi Exchange Tokens per visit
1 Token for Good Oral Hygiene
1 Token for Arriving on time
1 Token for Having Nothing Boken or Loose (for any reason!)
1 Token for Cooperation with Rubber Bands, etc.

**You will be rewarded 4 Tokens per visit as long as your hygiene is good.**

For Shorter Appointments and for Patients on Observation Recall:
(Such as an appt for Impressions, Separators, Retainer Checks, Appliance Checks, etc.)
Earn up to 2 Alborzi Exchange Tokens per visit.
1 Token for good oral hygiene
1 Token for arriving on time

**You will be rewarded 2 Tokens per visit as long as your hygiene is good.**
You can also Earn EXTRA Alborzi Exchange Tokens all year long…here’s how!!

  • Report Cards – We want to reward you for Excellence in Academics!
    Just bring us your current report card and earn 2 Tokens for an A and 1 Token for a B in the following core subjects: 
    Reading, Math, Science, English and Social Studies.
  • Office Contests - Earn up to 30 Tokens for winning our office contests.
  • Referring New Families - Earn 25 Tokens for referring a new family to our office (referral of immediate family members is not included).
  • Send us a postcard from your vacation and earn 1 Token.
  • Send us a postcard from the farthest destination and earn 15 Tokens!
  • Halloween Candy Buy Back – Earn 1 Token per pound of candy brought in.
  • Holiday Toy and Food Drive – Bring in a donation and receive 1 Token.
  • Watch for other opportunities and contests to earn MORE tokens all year long.

Alborzi Exchange Tokens may be cashed in at any time. Cash them in at each visit for a small toy or gift card OR save up for BIGGER prizes like CD players, iPods or a PlayStation 2.
